Social Services

Illnesses and injuries that require hospitalization can bring new changes to a patient and his or her family. Samaritan Hospital's Social Services employees work with the patient and the family to assist with discharge planning needs and continuity of care, which may include:

  • Community resources/referrals
  • Financial assistance
  • Specialized placement in a nursing home, rehabilitation facility or psychiatric care facility
  • In-home services/referrals
  • Home medical equipment supplies

Home Health Care

When a patient is homebound and requires skilled care, the physician may order home health services. Under the physician's direction, a nurse or other healthcare team member will visit the patient at his/her home to provide necessary treatment and services.

Durable Medical Equipment

Patients often require the assistance of durable medical equipment items, such as a walker, cane, bedside commode, hospital bed or wheelchair. These items can be used in the comfort of a patient's home or in a skilled care facility.

Extended Care Facilities

Extended care facilities may be needed for individuals who require more assistance than they can receive at home. A patient's length of stay at the facility may be short-term or long-term depending upon the patient's needs. Also, extended care facilities provide various services. Acute Inpatient Rehabilitation

Acute Inpatient Rehabilitation is a program that is designed to help individuals gain maximum functioning, while the patient is in the hospital. Patients who have experienced a loss of ability due to stroke, fracture, amputation, trauma or other disabling condition, may qualify for Acute Inpatient Rehabilitation. Under the direction of a physician, one or more of the following rehabilitative services may be required:

  • Physical Therapy
  • Occupational Therapy
  • Recreational Therapy
  • Speech Therapy
  • Dietetic Care
  • Social Work
  • Psychological Consultation

Outpatient Therapies

Patients who are not homebound but need rehabilitation services may be referred by their physician for:

  • Physical Therapy
  • Speech Therapy
  • Occupational Therapy
