Patients accessing services at any Samaritan Regional Health System facility or SRHS-owned physician practice will be asked for a photo ID to prevent identity theft.

This practice is in compliance with new regulations, called “Red Flag Rules,” set forth by the Federal Trade Commission (FTC). A red flag is a pattern, practice, or specific activity that could indicate identity theft.

As part of our proactive approach to prevent identity theft, a photo ID is required when opening or accessing an account and when a change of address request has been made.

Acceptable forms of identification include a driver’s license, state-issued ID card, passport or military ID, a Social Security card, a local utility bill, an employer or a school identification card.

Patient care will not be affected by this policy, as treatment will continue regardless of a patient’s ability to present acceptable identification. However, patients will be asked to bring appropriate documentation to their next visit.

Our staff has been trained to identify: suspicious documents; suspicious personal identifying information; and unusual use of, or suspicious activity related to, patient’s account. In addition, we may receive alerts, notifications or warnings from a consumer reporting agency or a notice of possible identity theft from third parties.
